Per license rules, I will comply with the NAD-RID Code of Professional Conduct and Licensure rules.

Per license rules, I understand I am not permitted to work in a medical, mental health, platform or performance settings unless teamed with a Generalist or Legal licensed interpreter. I also understand that I am not permitted to work in legal settings under any circumstances. (A.R.S. R9-26-501)

Per license rules, I will maintain journals for teaming/mentoring and submit documentations upon renewal.

Statutory Information for the Reference of all applicants

Sec. 3. Section 41-1030, Arizona Revised Statutes, is amended to read:

41-1030. Invalidity of rules not made according to this chapter; prohibited agency action; prohibited acts by state employees; enforcement; notice

G. An agency shall prominently print the provisions of subsections B,D, E and F of this section on all license applications, except license application processed by the Corporation Commission.

B,D,E and F:

B. An agency shall not base a licensing decision in whole or in part on a licensing requirement or condition that is not specifically authorized by statute, rule or state tribal gaming compact. A general grant of authority in statute does not constitute a basis for imposing a licensing requirement or condition unless a rule is made pursuant to that general grant of authority that specifically authorizes the requirement or condition.

D. This section may be enforced in a private civil action and relief may be awarded against the State. The court may award reasonable attorney fees, damages and all fees associated with the license application to a party that prevails in an action against the State for a violation of this section.

E. A State employee may not intentionally or knowingly violate this section. A violation of this section is cause for disciplinary action or dismissal pursuant to the agency's adopted personnel policy.

F. This section does not abrogate the immunity provided by section 12-820.01 or 12-820.02.S

• Documentation of name change if you applied under a name different from that on your documents (birth certificate, degrees, etc).

i.e.: Your last name is different than your current last name on any documentation

• Fill out and submit "Arizona Statement of Citizenship & Alien Status for Public Benefits" mandatory (submit passport, birth certificate, or valid work visa, etc)

• Two identical Passport Photographs (passport sized with date on back - taken no more than 6 months before date of application)

Directions on uploading passport photographs: Please scan your two (2) passport photographs side by side at high quality (at least 300 DPI). Ensure photographs line up straight at 90 degrees and upright (i.e. not slanted or taken at an angle distorting the photo). Before you upload, please make sure the photo is clear and sharp (i.e. not dark or blurry, etc.) with WHITE background and your face showing clear (i.e. no long hair over face, etc.). Upon review, if the photograph is not acceptable by the Commission, please be prepared to send us the hard photos in the mail.

* Note: You can inquire if the passport photo services can save the photos electronically for you on a CD or flash drive to upload instead.

Second option is to mail the two hard photos to ACDHH and staff will process them for you.

Please submit following documents along with your application:

• Education:

80 Hours - Photocopies of certificates of completion or transcript showing that you completed the required hours of interpreter preparation training from an accredited college or university or NAD/RID/BEI approved programs

A photocopy of a letter provided by NAD/RID/BEI indicating that you passed the written examination

• Work Experience:

Following hours of interpreting for which a license is not required under A.R.S. Section 36-1971(only pick one of the three following to be submitted and provide photocopies of your documentation)

i. A state-issued certification or certificate of competency in good standing

OR

ii. A score of at least 4.0 on the EIPA performance test

OR

iii. ACCI Certification (American Consortium of Certified Interpreter)
